Applications should be made on the appropriate formal Scout Association forms which can be found on The Scout Association website using the following links:

Good Service Awards

Meritorious Conduct and Gallantry Awards

However all applications must be sent to the Local Awards Advisory Group.  If any further information or clarification is required, you will be contacted direct.  It is important at this stage to keep the nomination confidential.
